Mindestens die dritte Region des englischen "Gesundheitsdienstes" NHS nun hat sich gegen die Verwendung von Firdapse™ ausgesprochen. Es wird festgestellt, dass mit 3,4-DAP Phosphat (= Firdapse™)  keine zusätzlichen klinischen Erkenntnisse  erbracht worden sind, alle Forschungsergebnisse basieren auf Studien mit 3,4-DAP Base. Das ist der Wirkstoff, der auch bei uns in Deutschland bisher in Kapseln oder Tabletten aus England so erfolgreich angewandt wurde.

DAS NHS Worcestershire Area will nur in begründet außergewöhnlichen Umständen und nur individuell auf Antrag de Kosten für Firdapse™ übernehmen.

"Amifampridine phosphate (Firdapse®) for Lambert-Eaton myasthenic syndrome (LEMS) is not recommended for use within NHS Worcestershire....Members noted that no additional clinical evidence in LEMS patients was produced to support the license application for Firdapse® and that all clinical evidence originated from studies of amifampridine base."

Vorweg eine Erinnerung:



Jahrzehntelang wurde vom Kaliumkanalblocker 3,4-Diaminopyridin (3,4-DAP) zur erfolgreichen symptomatischen Behandlung der Muskelschwäche  (nicht nur des) Lambert-Eaton-Myasthenischen Syndroms (LEMS) gesprochen. Dabei bezog man sich auf die chemische Bezeichnung aufgrund der Strukturformel. . Erst kürzlich legte man sich international auf die Bezeichnung Amifampridin fest, wenn von 3,4-DAP als Wirkstoff/Wirksubstanz die Rede ist.

Wenn eine Firma ein Produkt mit dem Wirkstoff (als Beispiel) Amifampridin anbietet, hat diese das Recht, einen Handelsnamen zu erfinden (= INN). Dieser Name ist geschützt, wenn hinter dem Namen ™ oder ® steht:

 

3,4-Diaminopyridin (3,4-DAP)
bezieht sich auf die chemische Strukturformel.

 

Amifampridin
ist die internationale Bezeichnung für den Wirkstoff 3,4-DAP.
Amifampridin wirkt als Kalium-Kanal-Blocker.

 

Firdapse™ bzw. Firdapse®
ist der Handelsname (= INN)des Produkts von BioMarin.
Die Tabletten enthalten als einzigen Wirkstoff Amifampridin,
= chemisch: 3,4-Diaminopyridin-Phosphat.

Stellungnahme des Staatlichen Gesundheitsdienstes Nord-Ost in Großbritannien

 

Das Institut empfiehlt, in seinem Bezirk statt 3,4-DAP  der Firma BioMarin, weiterhin  die bisherigen Präparationen von 3,4-DAP  zu verwenden.  Die extreme Kostensteigerung sei nicht gerechtfertigt, BioMarin habe iel immerhin mit Firdapse™ (noch) keine Studien durchgeführt.

 

 

 


 

 

[BioMarin konstruiert in seinen Informatioen a.a.O.  einen Gegensatz von "Firdapse™" und 3,4-DAP, der natürlich nicht existiert. Der einzige Wirkstoff in "Firdapse™ ist 3,4-DAP-Phosphat. Andererseits behauptet BioMarin in einer Informationsbroschüre jedoch, die Effektivität von "Firdapse™" habe sich in Studien erwiesen. Diese Studien wurden aber in der "Vor-Firdapse-Zeit" durchgeführt und bewiesen den Erfolg in der Behandlung des LEMS mit unlizenziertem 3,4-DAP.  Die Begriffe "Firdapse" und "3,4-DAP" werden also je nach dem verwendet, wie es BioMarin für sich argumentativ als vorteilhaft erachtet. National Health Service North-East - Treatment Advisory Group:

 

Amifampridine phosphate (Firdapse®) for Lambert-Eaton myasthenic syndrome is not recommended for use within NHS North East. The group did not consider amifampridne phosphate (Firdapse®) to be a costeffective treatment contingent on the continued availability of unlicensed amifampridine (3,4-diaminopyridine).

 

The group noted that Firdapse® is currently substantially more costly than equivalent doses of unlicensed amifampridine. The group noted the additional patient support and research programme that the manufacturer of Firdapse® has committed to. However, it was considered that the incremental cost of does not represent value for NHS North East.

Der extrem hohe Preis von "Firdapse™ erscheint dem britischen Gesundheitsinstitut (NHS North-East] als nicht gerechtfertigt,
weist jedoch auch (1] die rechtliche Sicherheit bei der Verordnung eines lizenzierten Produktes hin, (2) auf die Ankündigung von BioMari, neue Studien durchzuführen und darauf, dass BioMarin Informationen für Patienten zur Verfügung stellt.

 

 

No new clinical studies were conducted to support the license application for Firdapse® with all evidence obtained from the existing published evidence pertaining to amifampridine. A bioequivalence study in healthy volunteers demonstrated acceptable bioequivalence between amifampridine and amifampridine phosphate.

 

Firdapse® is substantially more costly than imported sources of
amifampridine, with annual drug costs per patient estimated at
about £30,000 to £45,000 compared with £1,500 to £2,200.

  • Firdapse® provides benefits over amifampridine due to being a

licensed medicinal product. These exist in terms of: patient
information, post-marketing safety monitoring, and assured
product quality and accuracy in dosing.

  • In addition, the manufacturer will be undertaking and supporting

further research into the management of LEMS.

  • Guidance from professional and regulatory bodies recommends

that licensed medicinal products are used in preference to
unlicensed products.
